Across imaging, surgery, and cardiac care, health tech's center of gravity is shifting from proving that AI works to proving it fits. Vendors are shipping AI into existing clinical workflows rather than beside them, regulators on both sides of the Atlantic are formalizing how AI-enabled devices get monitored after launch, and the biggest technology entrant of the period is a government contract for scheduling and intake, not a diagnostic breakthrough. The throughline: infrastructure, governance, and adoption discipline are becoming the real differentiators, not the algorithms themselves.

Regulation, Reimbursement and Market Access

  • FDA medical device user fees for FY 2027 rise nearly 10 percent across major pathways: 510(k) fees move to $28,653, De Novo to $191,020, and PMA to $636,732, continuing a decade-long trend that has taken the 510(k) fee up more than 500 percent since FY 2017
  • EU and US post-market surveillance for AI-enabled MedTech now run on structurally different logic: the EU anchors PMS in MDR and the AI Act with PSUR/PMSR cycles and Notified Body touchpoints, while the US FDA uses a total product lifecycle approach built around an authorized PCCP
